Waiver, Release of Liability & Informed Consent

1. Voluntary Participation

I acknowledge that my participation in any session, ceremony, retreat, or related activity offered by Kasey is entirely voluntary.

I understand that I may choose to discontinue participation at any time.

2. Nature of the Work

I understand that these services may include practices such as:

• Plant or natural medicines
• Breathwork, meditation, and energy-based practices
• Physical, emotional, and integrative processes

These experiences are intended for personal exploration, wellbeing, and self-development.

They are not medical, psychiatric, or psychological treatment, and are not intended to diagnose, treat, cure, or prevent any condition.

3. Assumption of Risk

I understand that participation may involve inherent physical, emotional, and psychological risks, including but not limited to:

• Nausea, vomiting, diarrhea, or other physical discomfort
• Dizziness, fatigue, or changes in bodily sensations
• Changes in perception or awareness
• Increased heart rate or physiological responses
• Emotional release, catharsis, or psychological distress
• Injury, illness, or other unforeseen complications

I acknowledge that risks may be known or unknown, and I voluntarily assume full responsibility for any and all outcomes arising from my participation.

4. Medical Disclosure & Responsibility

I confirm that I have fully and honestly disclosed:

• All medical conditions
• Medications and supplements
• Mental health history
• Substance use

I understand that certain conditions may pose increased risk, including but not limited to:

• Cardiovascular conditions
• Seizure disorders
• Bipolar disorder, schizophrenia, or other serious mental health conditions
• Pregnancy
• Use of SSRIs, MAOIs, or other contraindicated substances

I accept full responsibility for ensuring my readiness to participate and for providing accurate and complete information.

5. Fitness to Participate

I confirm that I am physically, emotionally, and psychologically fit to participate.

I take full responsibility for my wellbeing before, during, and after any session or retreat.

6. No Medical Advice or Treatment

I understand that Kasey and any facilitators:

• Are not acting as licensed medical providers unless explicitly stated
• Do not diagnose, treat, prescribe, or provide medical advice

I agree that I am responsible for seeking appropriate medical or professional care where needed.

7. Release of Liability

I hereby release, waive, discharge, and hold harmless Kasey, facilitators, assistants, volunteers, property owners, hosts, and any affiliated parties from any and all liability, claims, demands, actions, or causes of action arising out of or related to my participation.

This includes, but is not limited to:

• Physical injury
• Illness or medical complications
• Psychological or emotional distress
• Property loss or damage
• Financial loss
• Death

8. Indemnification

I agree to indemnify, defend, and hold harmless all parties mentioned above from any and all claims, demands, liabilities, damages, costs, or legal actions arising from or related to my participation.

9. Emergency Medical Authorization

In the event of an emergency, I authorize Kasey or designated facilitators to obtain medical assistance on my behalf.

I accept full financial responsibility for any medical care or services provided.

10. Personal Responsibility & Conduct

I agree to:

• Take full responsibility for my behavior, choices, and participation
• Respect the space, facilitators, and other participants
• Follow all guidance provided for safety and wellbeing

I am responsible for my personal belongings and for any damage caused by my actions.

11. Confidentiality

I agree to respect the privacy of all participants and to maintain confidentiality regarding any personal experiences shared during sessions or retreats.

12. Media & Image Release

I understand that photos, videos, or other media may be captured during sessions or retreats.

By proceeding with booking, submitting a form, or participating in any session or retreat, I confirm that I provide consent for my image, voice, or likeness to be used for promotional or educational purposes.

I understand that:

• No sensitive or private moments will be intentionally captured or shared
• Content will be used respectfully and in alignment with the nature of the work

If I prefer not to be included in any media, it is my responsibility to inform Kasey in advance or at any point during the experience, and this preference will be fully respected.

13. Governing Law

This agreement shall be governed by the laws of the State of Florida.

14. Severability

If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

15. Agreement

By proceeding with booking, submitting a form, or participating in any session or retreat, I confirm that:

• I have read, understood, and agree to this Waiver, Release of Liability, and Informed Consent
• I am participating voluntarily and accept all known and unknown risks
• I accept full responsibility for my health, wellbeing, and any outcomes arising from my participation
